Tell us what brings you in.

Whether it is time for a routine visit, something has changed or you are considering treatment, begin with the concern that matters to you. Mention any previous experiences or access needs that would help the team prepare.

What to have ready.

For your visit, prepare the information below. Use only an approved secure route for sending records; this review website does not accept medical documents.

  • Your health history and current medicines.
  • Information about allergies and previous dental care.
  • Insurance details, if you plan to use benefits.
  • Questions about your teeth, comfort or treatment choices.

Understand the first appointment.

A dental assessment generally begins with a conversation and an examination, with imaging when appropriate. The findings help shape the next steps. Cleaning may be appropriate at that visit or may need a separate appointment, depending on the care required.

Before agreeing to treatment.

Ask what is recommended, why it is being recommended and what alternatives exist. Discuss the likely appointments and an estimate for your plan. Sunbit financing can give eligible patients a way to pay over time. Confirm your insurance benefits and financing terms before you rely on them.

Planning a visit for your child.

Tell the team your child’s age, dental history and any comfort or support needs before the visit. Ask who will provide care and whether a pediatric dental specialist is appropriate. Our children’s care guide explains checkups, prevention and questions for parents.

When a concern needs prompt attention.

Tooth pain, an injury or a damaged tooth may need timely dental assessment. The Emergency Dentistry guide explains what to discuss with a dental professional. For urgent appointment needs, call (224) 567-8278. The online request form is not an emergency service.

Forms, records and appointment questions.

Approved patient forms and portal links will be added here when configured. An appointment request expresses your interest in a visit; it is not a confirmed appointment until the practice agrees the details with you.

Finding the office.

Visit us at 8830 West Dempster Street, Niles, IL 60714. Find our phone number and directions on the contact page, and call ahead to confirm office hours or any access arrangements you need.
