Start with the treatment plan.

The cost of care depends on what is recommended for you. Ask for an explanation of the proposed treatment, the estimated fee and any items that may be billed separately. If a plan changes, ask how that affects the estimate.

Insurance questions to clarify.

Participation and benefits are different questions. Confirm whether the treating provider participates in your specific plan, what that plan may contribute and what remains your responsibility. An estimate of benefits is not a guarantee of payment.

• Does the plan apply to this provider and treatment?
• Are there waiting periods, limits or authorization requirements?
• Which charges are outside the proposed estimate?

Payment questions to bring.

Before choosing how to pay, ask about the total cost, any interest or fees, payment schedule and the terms of a financing offer. Review status: accepted payment methods, insurance participation and Atlas-specific application details still need confirmation. Financing approval and terms are determined through the provider’s application process.
